When you log in, you will be at your homepage. There are four main sections: benefits, resources, life events, and company information. In the benefits section you have access to benefit summaries, SPDs, links to provider networks and insurance carriers and help with understanding benefits. In the resources section you will find health and wellness information, interactive financial planning tools and information on state and federal programs. In the life events section, you will find all of the forms you need if you are going through a life event such as marriage, birth or divorce.

Medical & Dental Coverage Change Forms can be picked up at Human Resources or downloaded here. If you will be declining coverage, Remember changes to your policies can only be made during “open enrollment” or throughout the year with a “qualifying event”. - What are the qualifying events for changing my insurance coverage?
Change in Status (see pages 5-6 of the Summary Plan Description for a complete list) such as (not all are listed);
- Spouse/dependent’s lose their benefits
- Divorce, separation, child support order
- Death
- Childbirth, adoption, stepchildren
- Marriage
- Spouse/dependent becomes eligible for Medicare/Medicaid
A LL OF THESE ARE SUBJECT TO THE 31 DAYS PERIOD OR EMPLOYEE MAY BE SUBJECT TO “OPEN ENROLLMENT” RULES.
- What does level of coverage mean?
If you have Single coverage, you can ONLY Change in Status within the 31 days of the qualifying event. If you wait beyond the 31 days-even if it is a “qualifying event”-you will have to wait for open enrollment, December 1-31 to be effective January 1.
If you have Family coverage, you can make change (except going to Single coverage) after the 31 day period, but changes will be effective the month following receipt of the Change Form (example: newborn coverage not going back to DOB)
- What if I have Single medical/dental and I have a baby, is the baby automatically covered on my insurance?
NO, you must complete a change form for medical/dental to cover your baby. We cannot make any changes to your policy without your written permission.
- How does “open enrollment” affect newborns?
If you do not add the baby within 31 days from birth—you may not be able to add baby until December 1-31 for coverage effective January 1. Reasons are as follows:
**Adding baby within 31 days, baby is covered from date of birth. You can go from SINGLE to FAMILY within the 31 days from qualifying event.
**If you wait beyond 31 days you will be able to add baby effect the 1st of following month ONLY if you already have FAMILY COVERAGE!!!
**If you currently have SINGLE COVERAGE and you do not add baby within the 31 days from birth—you will not be eligible to add baby until “open enrollment” December 1-31 to be effect January 1.
**If you do not have any coverage and have a baby, you can get FAMILY coverage within 31 days.
- What if I have Single coverage and I get married, but forget to complete a Change Form until 2 months after the wedding, can I add my new spouse?
Not until December 1-31 to be effective January 1. Be sure and turn in a Change Form in December!! No Single to Family unless within the 31 day qualifying event period.
**If you do not have any coverage and you get married, you have 31 days to apply for FAMILY coverage.
- Can I get a benefits manual?

- What date does my insurance stop if I term out?
We (LLBO) covers the employee through the rest of the month at full cost. i.e.; If an EE terms out on January 3, LLG continues to pay CCStpa until January 31. If you, as the manager, know you are going to terminate someone’s employment on the 28th of the month, please complete the paperwork and term them so as not to cause this extra expense to the company.
