The Leech Lake Elderly Advocate will assit you with the following:
- Home visits with elder/disabled
- Assistance with completion of any type of forms such as Medicare Medicaid, Food Support, etc.
- Help to optain needed medical equipment such as wheelchairs, walkers, ramps, canes, etc.
- Home Health Care/ PCA
- Safety checks and fall prevention education
- Referral to other agencies that may be of assistance to you
- Help with attaining Eyeglasses, dentures, hearing aids
- Trips to tribal events
- Transportation information
- Limited trips for shopping and banking
- Limited chore service
- Good listening ear
Eligibility Requirements
- Enrolled in a federally recognized tribe
- Age 55 or older or disabled
- Live on or near the Leech Lake Reservation
- No income requirements
