We provide eyeglasses, dentures, hearing aids and meet miscellaneous non-recurring needs to eligible applicants.
Eligibility
- Leech Lake Band Member
- 55 years of age and older
- Have a disability
- The applicant has limited income resources to pay for these services, i.e., receives RSDI, MFIP, VA, etc., monthly pensions.
- The applicant does not have or is not eligible for Medical Assistance or any other program that may assist in providing these services.
Documents Requested
- Proof of enrollment
- Proof of age
- Proof of disability - defined by Social Security Disability and/or by a certifying physician
Program Requirements
- Eyeglasses and Dentures must be purchased through the Indian Health Services, Cass Lake Indian Hospital
- Hearing Aids must be purchased through the Audiologist Department at the Indian Health service, Cass Lake Indian Hospital.
- Special circumstances will be considered to all applicants receiving services from the Elders Advocate Program.
Spending Limits
Applicants will be limited to assistance for eyeglasses. One (1) set of eyeglasses every two (2) years. The Elders Advocate Program will pay up to $125.00 per set of eyeglasses. All expenses above $125.00 will be the responsibility of the applicant.
Applicants who are applying for dentures will receive up to $333.00 for uppers and up to $333.00 for lowers. The Elders Advocate Program will assist with purchasing sets every four (4) years. All expenses over the above mentioned funding limit will be the responsibility of the applicant.
The Elders Advocate Program will pay up to $175.00 per crown, up to $800.00 for procedure. All added expenses will be the responsibility of the applicant.
Applicants applying for Hearing Aids will receive up to $300.00 for each hearing aid, limit two (2). All added expenses will be the responsibility of the applicant.
