This program is based out of the Family Preservation Program and is available to Foster Care Children that have aged out or will be aging out of Foster Care. We assist young adults to make the transition to self-sufficiency by providing assistance to :
- Gain employment
- Enter post secondary training and eduational institutions
- Provide personal and emotional support through Family Preservation Program Counseling
- Provide financial and other appropriate supportive services
Supprotive Services Include
- Driver's training
- Driver's License or State ID
- Vehicle repairs
- Car insurance
- Job interview/work clothing
- Gas vouchers for transportation to SELF related activities
Activities include
- One-on-one meetings
- Traditional Teachings
- Educational Field Trips
- Conferences
- Retreat
Eligibility
- Native American youth age 14-17 who:
- are in a Tribal approved out-of home placement and likely to remain in placement until age 18 or older
- Developmentally disabled youth capable of living independently in the community after aging out of tribal approve out-of-home placement.
- leave an out-of-home placement after age 16 for adoption, kinship care, or legal guardianship.
- Young adults age 18-21 who have aged out of a tribal approved out-of-home placement.
