Each new recipient is required to meet with a therapist to complete a Diagnostic Assessment.
Should Medication Management be deem necessary an in-house referral shall be made via the Administrative Assistant who will schedule an appointment with either Dr. Wildgen (adults) or Mental Health Therapist (minors).
Eligibility Requirements
Anyone who has been determined to be eligible to receive services through Indian Health Service. Service recipients can be referred by a medical provider or medical facility, court-ordered, and/or self referred.
It is the recipient’s responsibility to contact the program to arrange needed transportation, cancel appointments as well as to reschedule if needed.

Intake Procedure
- All recipients must first complete an intake packet and return it to the Administrative Assistant before initial appointment. Intake packet includes:
- Consent for Treatment form
- Insurance information (insurance NOT necessary)
- Shared Information form
- Problem Checklist form (2 pages)
- Receipt of Clients Rights/Responsibilities
- Data Privacy
- Legal Requirements & Consequences
- Therapist’s Competence
- Listing of other Mental Health Resources available in the area.
- When intake packet is completed and returned to the Administrative Assistant, an
appointment will be scheduled at the earliest appointment possible time - Service recipient will be sent an appointment reminder.
