Medical Transportation Program

The Leech Lake Band of Ojibwe has partnered with the Minnesota Department of Human Services through a Health Care Access Plan to offer you transportation to medical providers and medical transportation reimbursement services through our CHR program.

There are three components to this plan:

Medical Transportation Program (MTP)

This program is offered through the CHR program of the Health Division. CHR employees provide transportation for you in Tribal Vehicles to your medical appointment. Eligibility for this program are Leech Lake Band members, Band members from other reservations, and non-band members.

Services are provided in order of priority in the following order:

  1. Elders
  2. Diabetics/Dialysis
  3. Chemo/Cancer Patients
  4. Heart Patients
  5. Disabilities
  6. Remaining Population

Eligibility Guidelines:

  • Enrolled in Minnesota Health Care Program
  • Not Enrolled in a Health Care Program

Medical Transportation Reimbursement Program (MRP)

This program is offered through the Health Division CHR department and is a plan that will:

Pay for the most cost effective way to get you to the closest provider with the level of care you need.

This reimbursement will pay for you to drive your own vehicle, have a family member take you or a friend. Eligible reimbursement cost includes mileage at a rate of .20 per mile and parking fees, as well as, lodging and meals if your required medical attention isn't available locally. If you are required to have someone go with you (such as a mother of a child) those expenses are also eligible for reimbursement. This program does not have a priority list.

Steps to take to receive this service:

  1. Call the CHR department 24 hours before your appointment.
  2. Complete the Travel Log and have your provider sign it.
  3. Save all receipts for expenses and turn them in with the Travel Log.

Eligibility Guidelines:

You MUST be enrolled in Minnesota health Care Program.

If you are not enrolled and desire to have help in completing an application please call the health division Benefits coordinators at 335-4500 or 335-4536 or stop by the Health Division for assistance.

Additional Information for MRP:

This program is designed for friends, family or a member of the general public to assist a MA participant to get to their medical appointment. This program allows for the driver to notify the CHR Department that they are transporting an MA Participant. The driver then comes into the Health Division and picks up a blank ride sheet which the provider has to sign to confirm they attend the appointment. Once the ride sheet is complete they turn it into the CHR Department for reimbursement. This program reimburses mileage at .20 per mile as well as parking fees.

This program will also reimburse meal costs for long distance trips at the rate of: Breakfast = $5.50; Lunch = $6.50; & Supper = $8.00. Receipts are required to receive reimbursement.

Medical Volunteer Driver Program (MVP)

This program is offered through the Health Division CHR department and is a new program offering medical transportation by a registered volunteer driver, who will transport you to the closest provider capable of providing the level of care you need. This program pays the volunteer driver directly (Reimbursed at .51 per mile) and the service is provided at no cost to you. If you are required to have someone with you (such as a mother of a child) this program transports you at no cost as well. This program does not have a priority list.

Steps to take to receive this service:

Call the CHR department 7 days before your appointment to register and be schedule for transport.

You MUST be enrolled in Minnesota Health Care Program.

If you are not enrolled and desire to have help in completing an application please call the health Division Benefit Coordinators at 335-4500 or 335-4536 or stop in for assistance.

If You Desire to be Employed as a Volunteer Driver

If you are an individual with a valid drivers license and have a vehicle with valid insurance and you desire to be employed as a volunteer driver with our program please call the CHR coodinator at 335-4507 and request an application.

Additional Information for MVP:

These drivers only transport patients assigned by the CHR director. Volunteer drivers cannot transport family members unless assigned by the CHR Director.

Bus Ticket Program

This program is designed for MA participants to call into the CHR Department and request a round trip ticket which will allow them and their child (if the child is the patient for the appointment) to ride the Leech Lake Transit to their appointment. Each participant will be responsible for turning in the appointment confirmation sheet into the CHR Department after the appointment has been attended.
