IMPORTANT INFORMATION

Changes in the Medical Plan Effective August 1, 2011

Remember changes to your policies can only be made during “open enrollment” or throughout the year with a “qualifying event”.  A summary of the qualifying events can be found in the Leech Lake Band of Ojibwe – Premium Only Plan Summary document.  This can be located on our benefits website at pcms.plansource.com User ID:  leech and Password: lake

REMINDER: Update Your Beneficiaries

Changes to your beneficiaries (for life insurance or retirement plan) can be made throughout the year. Beneficiary Change Forms can be picked up at Human Resources or from the Benefits web page.
